Community Connector – Tower Hamlets

 
The Tower Hamlets Community Connectors service is a partnership between Mind in Tower Hamlets and Newham and East London NHS Foundation Trust.
Community Connectors are located across the borough of Tower Hamlets each focusing their support to people registered in network areas.

Our Community Connector focuses in Network 9 which is a part of the Stepney and Wapping Community Mental Health Team (CMHT).

This is a multi-disciplinary team of health care professionals that includes:

  • Social Workers
  • Nurses
  • GPs
  • Employment Advisors
  • Psychiatrists
  • Social Prescribers

How we help

The role of Community Connectors is to help people with serious mental health issues such as personality disorders, schizophrenia and other severe mood disorders to prevent the onset of a mental health crisis by enabling quick and easy access to appropriate support.

Community connectors in partnership with their multi-disciplinary team offer help to:

  • Identify support needs
  • Receive a multi-disciplinary team care plan
  • Reduce social isolation
  • Access benefits and housing advice
  • Receive emotional support
  • Access employment support

Accessing our service

This service is for adults who reside in Tower Hamlets (Stepney and Wapping) who are experiencing serious mental health issues.

The service can be delivered remotely or face to face at the Mind in Tower Hamlets office, Stepney and Wapping CMHT office or selected community locations in Tower Hamlets.
