These specialist Community Connectors support adults over the age of 65 with serious mental illnesses or complex emotional needs.
While fulfilling the same role as other Community Connectors, they are particularly able to address the many barriers that isolate older adults. They support the social inclusion that is vital to wellbeing.
Breaking down barriers
Older adults Community Connectors listen to each person and take a hands-on approach to support. This includes things like:
- accompanying someone to a wellbeing group until they have built the confidence to go alone
- helping someone access a PHB to buy clothes so they can leave their home without embarrassment
- applying for a taxi card or mobility scooter to give someone pain-free access to activities
