We provide several Community Connectors who work with the NHS.
Community Connectors help people with serious mental illnesses to access the right support within the community. They form part of the NHS multi-disciplinary team (MDT), consulting with clinical staff and supporting each person to address their individual challenges.
We have a different perspective to the clinical team. We take a holistic approach, thinking about all of what the person needs. And we work more closely with the individual, with more contact.

How community connectors can help
Our Community Connectors work with people to identify the wider issues that impact on their health and wellbeing, such as:
- debt
- poor housing
- employment circumstances and unemployment
- loneliness and isolation
- caring responsibilities.
They provide practical support so the person can address those issues. Here are just a few examples of what they might do:
- help someone contact the council about the state of their home
- connect the person to social groups and activities
- organise events, classes and get-togethers
- research affordable adult learning courses to suit the individual
- signpost to immigration support services or financial advice.
Support in a community setting
While based in the office, Community Connectors can also meet people in their own homes, or in a safe place like a library or café. It provides a comfortable, non-clinical setting, and allows face-to-face support for people who struggle to leave the house.
